Annual Affiliate Dues and Commitment Form
As stated within the FASD United Affiliate Guidelines, annual dues are based upon the affiliate organizations annual operating budget. The schedule for dues is based upon the portion of FASD-related program income from the previous year and based upon the following schedule. Annual dues are payable to FASD United by March 31st of each year.
$0-$50,000 FASD-Related Income = $30 Annual Dues
$50,001-$100,000 FASD-Related Income = $60 Annual Dues
$100,001-$250,000 FASD-Related Income = $120 Annual Dues
$250,000 and above FASD-Related Income = $240 Annual Dues
