| It’s time to renew Your Affiliate Network membership. Each Affiliate Organization will have been sent a renewal notice to the primary contact listed on your account. If you haven’t received an email, please check your spam, or email elsworth@fasdunited.org to have it resent. Renewal in the Affiliate Network this year is as easy as 1, 2, 3. 1. Fill out the Annual Commitment Form A link to this form was emailed to the primary contact on the account. Unsure who this is? Contact elsworth@fasdunited.org. 2. Pay Your Dues After the Commitment form is completed, an invoice that can be paid online or by check will be emailed to the Affiliate primary contact. 3. Get Confirmation and Communication Once your organizations dues payment is received you will receive confirmation. Anyone you’ve listed on your commitment form will receive a login and password to the affiliate website as well as communications about upcoming Affiliate Network events and activities.
